In the upper extremity, traumatic segmental bone loss beyond the limits of conventional bone graft often is associated with other injuries which justify amputation. Vascularized bone transfer is more often indicated in tumor than trauma surgery, and more often for late than acute reconstruction, for segmental defects longer than six centimeters.
