Significant bacterial contamination has been defined as greater than 100,000 bacteria per gram of tissue. However, high energy open fractures which require flap cover tolerate less. Quantitative tissue cultures are more reliable than swab in this context. If culture results from the time of flap closure are high, the flap may be re-elevated for redebridement at 48 hours.
