Dorsal PIP fracture dislocation therapy

0 - 3 days:

A variety of splints may be used depending on surgeon's preference. The goal of splinting is to block 30 degrees of extension and allow full flexion. This may be accomplished with a dorsal blocking splint, a ring splint or with buddy taping. Alternatively, a static 30 degree extension block may be used, but ideally the finger should be allowed to flex actively.

The postoperative dressing is removed.

Active Range of Motion exercises are initiated 15 min/hr. within the splint.

2 weeks:

Passive Range of Motion exercises are initiated within the splint.

3 - 6 weeks:

The Dorsal Blocking splint is adjusted to increase PIP extension 10 degrees each week.

6 weeks:

The Dorsal Blocking splint is discontinued.

Active and passive extension exercises are initiated to the PIP and DIP joints.

Extension splints may be applied as needed.

The patient should have protective splinting worn during sports level activities until 12 weeks after injury.
